Before you begin the online application: We recommend that you review this PDF version of the application format, so that you can prepare some of the responses in a word processing program first, to avoid any time-out issues. You can then go to the online system and cut and paste the responses, saving as you go.
The application collects general contact information for you and your parent(s). Your address, phone numbers and email address will only be used to communicate with you regarding the SHAD program.
Your application is confidential and will be used solely for selection purposes. SHAD does not share or otherwise profit from the distribution of personal data beyond the use it was provided to fulfill.
LETTER OF REFERENCE
Each applicant must solicit a letter of reference, to be submitted through our on-line form (available at www.shad.ca/reference) completed by either their Principal, a teacher or Guidance Counsellor (someone who can address your qualities aligned with the type of student we are looking for). Within the online application, you will need to provide the reference contact's name.
IMPORTANT: Remember, it is your responsibility to ask your reference to complete the online reference form.
Only one reference is required. If you wish to ask another person to complete a reference for you (another teacher, an employer, etc.), that is optional. The maximum number of references allowed per applicant is 2.
Note: The reference is confidential and will be used solely for selection purposes for the SHAD program.
Your course data must be certified by a school official (a signature is required).
The Academic Performance form must be completed even if a transcript is submitted with your application.
We reserve the right to request official transcripts to clarify your academic status.
If there is not enough room in the grids, you may include an extra page.
Current school year section: If you do not have marks yet for your current courses, still complete this section, so that we know what courses you are/will be taking. Interim or mid-term marks are fine.
IB students: Most IB course marks are criterion-based, graded on a scale from 1-7. Some schools provide a conversion table in order to convert these to equivalent high school percentages. You may use the IB 1-7 coding, but please also include your school's conversion table, if available.
Return the completed Academic Performance form by one of the following options:
Uploading the file within the online application system (NOTE: only 1 file can be uploaded, but the file may contain more than 1 page)

INVOLVEMENT IN VARIOUS ACTIVITIES
Your Personal Accomplishments Profile is a succinct description of your most recent and relevant experiences, interests and accomplishments. What you do outside the classroom demonstrates your initiative and drive, your leadership and interpersonal skills, and your character. Through this section of the application, we are trying to get an idea of the types of activities you are involved in, and the amount of time you devote to these activities.
There are 4 categories to summarize your involvement in various activities:
Describe the level and extent (time commitment and leadership roles) of your activities in school and the community (e.g. clubs, student government, Duke of Edinburgh, etc.)
Describe any significant academic awards you have earned in the last 3 years (e.g. Pascal, Cayley, DECA, science fairs, other awards)
Describe your involvement in the arts (e.g. music, dance, etc.) and athletics
Describe what type of work (paid or volunteer) you are involved with, and why
For each of the 4 categories you may include multiple activities within the category. (For example in the arts & athletics category, you could list any instruments you play, teams you are on, if you were in a school play or choir, etc.) It would be best to limit the timeframe to be within the last 3 years.
Please provide estimates:
the dates from and to (use a future date if you will be doing these types of activities into the future);
in a typical week, how many hours you spend on these activities
in a typical year, how many weeks you spend on these activities
Note that you are limited to a 255 character description/detail.
Please give serious thought to this key part of the application, as you have only 2,500 characters maximum (including spaces) to demonstrate your passion and your writing skills.
Choose one of the following topics:
Describe the impact you have had in participating in community activities.
Describe your best experience working as a leader or working in a group or team.
PERSONAL INNOVATION AND CREATIVITY IN ACTION
Demonstrate to us that you have the spark to be an innovative and creative problem solver. Explain your process: how did you go about solving a problem or creating something new? (2,500 characters maximum including spaces).
The goal is for you to explain how you approached a solution to a specific problem. Describe how you think about problem-solving by exploring a specific example. If your answer to the description requires you to show pictures, video or other media then you should include it in the optional Demonstration section, which must be used only to make the statement of creativity and innovation more clear. (If you have clearly expressed your creativity and innovation through the description, there is no reason to add content to the Demonstration section.)
For example, if you wish your problem-solving for the Creativity and Innovation description to be about composing music, then describe how music is a creative and innovative endeavour for you and also include the results of your composing in the Demonstration section. Alternatively, if you consider your creativity and innovation to be best captured in poetry, you would not necessarily need to include a sample in the Demonstration section.
You can further support your example of personal innovation and creativity from the previous section by providing a link or uploading a file.
Indicate if the material is original or from other sources.
If you have a video, you are limited to 1 minute maximum. Do not upload the file; rather, provide a YouTube link (recommended to be a public link, and tested to ensure it works!)
If you are uploading a file, a PDF is preferred, and there is a limit of 5 pages maximum.
