UW Shad Alumni Scholarship Consideration Form

Please complete this form by March 31, 2010, so that we may confirm with the University of Waterloo that you are a Shad Valley alumni and that you wish to be considered for one of their Shad Alumni entrance scholarships. Note that you will be automatically considered for the majority of entrance scholarships offered by the University of Waterloo if you have completed an application for admission through the normal university application process. Intent and Use of Form: The purpose of this form is to notify the Shad Valley office of your intended application(s) towards one or more of the five participating UW Faculties. This information is collected only for confirmation of Shad alumni status and, to this end, will be shared with the University of Waterloo for consideration for a Shad Alumni entrance scholarship. If you have any questions or concerns, contact scholarships@shad.ca for more information.
